I am the parent or legal guardian of the child(ren) attending and give permission for them to participate in Grace Orthodox Presbyterian Chruch’s (GOPC) Nature event, including hiking, nature exploration, crafts, and all related activities. I understand that I (or another adult I have named on this form) am responsible for supervising my children during the event.
Risks & Assumption of Risk:
I understand that this Nature event includes outdoor activities. Like any outdoor program, there are ordinary risks of minor injury, scrapes, insect bites/stings, sun exposure, allergic reactions, overexertion, uneven surfaces, or illness—including but not limited to those listed. I voluntarily accept these risks on behalf of myself, my child(ren) and family members.
Release
:
In consideration of my, my children’s and my family member’s participation, I release GOPC, its pastors, officers, volunteers, and employees from any claims for injury, illness, or loss that I, my child(ren), or family members may have, to the extent permitted by law, and agree to indemnify and hold harmless GOPC from any claims brought by me or on my behalf.
Medical Consent
:
I authorize GOPC volunteers or staff to seek routine or emergency medical treatment for me or my child(ren) if needed, including administration of first aid or medications as needed. I agree to be responsible for any medical costs.
Photo & Video Consent
:
I grant permission for GOPC to use photographs, short videos, or recordings of me or my child(ren) taken during the event in church print materials (such as bulletins, newsletters, or promotional brochures) and/or online (such as the church website, social media, or promotional videos). No names or other personal identifiers will be used unless separately approved.
